Community Health Advocacy

Community Health Advocacy
Information, Referral and Outreach Services assist people with MassHealth and other health insurance enrollment options. Springfield Partners also works to publicize the importance of having a primary care provider, which reduces emergency room visits. We can also help residents complete or submit food stamp (SNAP), fuel assistance, and other applications.

Our goals are to:
  • Increase community knowledge of health care resources
  • Improve community access to health care services
  • Increase community participation in health education programs
  • Refer women to free mammograms, available through the American Cancer Society
  • Improve community satisfaction with health care services
  • Maintain contact with people whose health-related problems put them at risk


Community Health Advocacy services are available at 721 State Street from 9 a.m. to 4 p.m., Monday through Friday.
