Asthma Outreach

Our Asthma Outreach Specialist provides follow-up visits to those who have been hospitalized for asthma. The specialist meets with the patient and his or her family to identify asthma triggers in the home and create strategies for alleviating or removing those triggers.

Our advocate:
  • Helps remove or reduce environmental triggers from the homes of asthma sufferers
  • Partners with the Pioneer Valley Asthma Coalition to offer educational events and collaborative programs to schools in Springfield and Holyoke
  • Works towards improving indoor air quality in schools, child care settings and workplaces
  • Gives patients referrals to visiting nurses and other healthcare services, and gives other referrals as needed

READY: Reducing Ethnic/Racial Asthma Disparities in Youth
READY (Reducing Ethnic/Racial Asthma Disparities in Youth) is a partnership of Baystate Medical Center and Springfield Partners for Community Action, funded through a grant from the National Institutes of Health. Through a closed referral system, READY aims to provide education and prevention services to about 100 families regarding asthma, and to facilitate access to essential services, including asthma management and treatment and case management.

Through this partnership, two Community Health Workers have been hired to:
  • Recruit and enroll families to the program
  • Provide social support to families
  • Provide asthma-related education and prevention services to enrolled families
  • Facilitate families' access to asthma management and treatment
  • Help develop strategies for identifying, approaching, educating and providing health education services to at-risk populations
