Our Alzheimer's Program provides case management for clients afflicted with Alzheimer's disease and for their families and caregivers.

More than 80 people suffering from Alzheimer's, primarily Latinos, were provided with information, referrals and support last year.
Our bilingual, bicultural Program Coordinator, Elsa Dones, has been providing these services for 8 years. The overall goal is to ensure that clients get appropriate care, and that family members know how to deal with the challenges of this disease in an appropriate manner.
With a nurse from the VNA, the program coordinator visits referred clients to help with evaluations, translations, and in providing information about the disease to client and family.

Anna cares for her grandmother, Sara, her husband and four children, and holds down a full time job.
Springfield Partners received a referral about Sara from a local health clinic. Visiting the family over a period of weeks, our Program Coordinator, Ms. Dones, explained the need for services, educated the family about the signs and symptoms of Alzheimer's, and discussed ways to deal with Sara's challenging behaviors.
Ms. Dones enrolled Sara in an adult day care program that provided meals and transportation, and allowed Sara to socialize with other seniors in a safe and comfortable environment.
Ms. Dones also arranged for a Personal Care Attendant (PCA) to help Sara with bathing and grooming. The PCA was also available to drive Sara to her doctor's appointments, allowing Anna and her husband to remain at work.
This assistance helped lessen Sara's aggressive behavior and helped the entire family better cope with her disease.
